Recently, a warning included in the prescribing information for the influenza medication baloxavir marboxil has sparked discussion among healthcare professionals and patients. The label advises against taking the drug with dairy products, calcium-fortified beverages, antacids, or oral supplements containing high levels of cations – specifically calcium, iron, magnesium, selenium, and zinc.

“Baloxavir marboxil is administered in a specific way, and metal ions like calcium, magnesium, iron, and zinc can cause chelation, forming compounds that are difficult for the body to absorb, significantly reducing the drug’s effectiveness,” explained Sae-rim Lee, a pharmacist and deputy director of the pharmacy department at Southwest Hospital of the Third Military Medical University, during a recent interview on December 11.

Many common products, including milk, fortified beverages, and certain medications and health supplements, contain these ions. Therefore, taking baloxavir marboxil with beverages like milk or soy milk is not recommended. Purified water or boiled water are considered the safest options for administration.

The body absorbs baloxavir marboxil over approximately four hours after oral administration, with peak drug concentrations in the bloodstream occurring around that time, marking the completion of the primary absorption phase.

Consequently, it’s advisable to avoid consuming dairy products, calcium supplements, or any supplements or oral medications containing aluminum, magnesium, or calcium within four hours of taking baloxavir marboxil. This precaution aims to maximize the medication’s absorption and ensure optimal therapeutic effect. Understanding how medications interact with common dietary components is crucial for effective treatment and patient safety, especially during periods of increased viral activity.
