A man sought treatment at an allergy clinic due to a burning rash on both hands, particularly on his fingers, and a blister on his left thumb.
Doctors quickly diagnosed the condition after learning he had hand-squeezed the juice from 12 limes to make margaritas and then spent the day outdoors watching a soccer match. His case was detailed in a report published in The Novel England Journal of Medicine.
A Case of “Lime Disease”
The man was experiencing phytophotodermatitis, also known as “lime disease.” This condition involves an abnormal or exaggerated skin reaction following exposure to sunlight after contact with a plant or fruit.
In this instance, lime juice was the culprit. The skin reaction is most often caused by plant substances called furocoumarines, which become toxic after irradiation by ultraviolet rays. It’s an inflammatory reaction similar to a sunburn, but it occurs several hours after contact with the plant.

The physicians’ report includes photos of the patient’s hands, showing red skin and a blister on one thumb – lesions similar to those from a classic burn.
There is no curative treatment for phytophotodermatitis. The condition typically resolves on its own, but this can seize weeks or even months. The first step is to wash the affected skin with soap and water to remove any traces of the triggering substance. Doctors may prescribe treatments to relieve symptoms and reduce the risk of infection.
This patient was treated with topical corticosteroids in the form of a cream applied to the lesions, and with a moisturizing cream to alleviate skin dryness.
His hands returned to normal after several months. He was fortunate, as this condition can sometimes cause hyperpigmented scarring on the hands that can take years to fade. The initial swelling and redness can give way to pigmentation that may be long-lasting.
