The Hispano-Uruguayan icon began his journey in Montevideo, where he played for Club Nacional de Football. During his tenure with Nacional from 1948 to 1957, he secured five national championships, establishing himself as a premier talent in South America.

In 1957, at age 28, Santamaría moved to Spain to join Real Madrid. He immediately became a vital component of the squad, making 34 appearances across La Liga and the European Cup in his debut season and winning both competitions. Over the next nine years, the legendary defender amassed 227 appearances and two goals, lifting a total of 12 titles, including four European Cups.

As a figure of Real Madrid in the 1960s, Santamaría also achieved the rare feat of representing two different national teams. He earned 20 caps for Uruguay between 1952 and 1957 before switching his international allegiance to Spain, where he earned 16 caps from 1958 to 1962.

Following his playing career, Santamaría transitioned into management. His coaching resume included a tenure with Español from 1971 to 1977 and multiple roles within the Spanish national team setup, including the youth and Olympic teams (1968–1971), the U21 side (1978–1980), and a two-year spell as the head coach of the senior Spanish national team from 1980 to 1982.
